Swim Lesson Provider Network Application

Description

USA Swimming's Swim Lesson Provider Network supports youth learn to swim programs across the country.  Organizations wishing to apply for participation in the Network must be willing to meet and maintain the following minimum requirements:

  • Utilize a nationally recognized learn-to-swim curriculum OR submit a copy of an independently-developed curriculum for approval
  • Certified learn-to-swim instructors and on-deck lifeguards holding current CPR and First Aid credentials
  • Maximum 6:1 student to instructor ratio for group lessons which serve young and/or beginning swimmers
  • Minimum of four (4) hours in-water instruction per child per session, or one class per week for month-to-month programs
  • Proof of current learn-to-swim insurance with minimum limits as follows:
    • Each Occurrence:  $1,000,000
    • General Aggregate:  $3,000,000
  • Written and practiced Aquatic Safety and/or Emergency Action Plan
  • Program facility is compliant with the ADA and regulated by the Department of Health or similar governing agency
  • Child protection & safety practices in place for staff and volunteers, to include:
    • Regular criminal background checks for those 18 and older
    • Child abuse prevention, response, and reporting training

Documents needed to complete the application process:

  • Federal Tax ID number, or social security number; depending on organizational structure
  • Complete, signed W9 form
  • General Liability Insurance policy effective dates and a copy of the current certificate of insurance (COI)
  • Aquatic Safety or Emergency Action Plan
  • Curriculum or progressions document for programs utilizing an independently-developed curriculum
  • Addresses, contact information, and website for all facilities
